Tri-Fold brochure

Задание и работа на английском:



I need one Tri-Fold brochure (8.5" x 11") based on the material of the document "To Design" (see attached). 



The style – colors, fonts, elements of design – should match the samples of design (will be provided). 



I understand, it has much info, but all information must fit. Also, it necessary to use images and icons to make it more attarctive. Moreover, I need some empty space at the end for additional doctor section that will be added later (10 – 15 doctors). Source (AI or PSD) files required.
