Documents include:
• Photo page of my passport
• Passport pages with Australian arrival stamps
• And (possibly, figuring that out now) health insurance card
For each document, the person signing will have to declare “I have sighted the original document and certify this to be a true copy of the original”, sign each page, list their name, date of certification, and contact phone number and position number (if relevant) and have the stamp or seal of the Authorised Officer applied.
